1 Procurement Details:
(1) Item Classification Number: 22.
(2) Purchase Subject and Quantity: Pharmacy Department Medical Devices 3 items: 1) Fully automatic weighing and dispensing powder packaging machine, 1 set; 2) Fully automatic tablet PTP dispensing machine, 1 set; 3) Automatic injection drug dispensing system, 1 set.

(3) Requirements for bidders: Bidders wishing to participate must submit sealed bids with documents proving capability to supply the items by the deadline. Bidders must respond to explanations requested by the responsible officer before the bid opening date.
(4) Invalid bids: Bids from ineligible participants or those failing obligations will be invalid.
(5) Contract document: Required.
(6) Award method: The bidder who submits valid bids with documents proving capability and offers the lowest price within the estimated price limit will be awarded the contract.
2 Eligibility for Participation:
(1) Must not fall under Article 5 of the National Hospital Organization Contract Regulations. Minors or persons under guardianship with necessary consent may apply under special reasons.
(2) Must not fall under Article 6 of the Regulations.
(3) Must have Ministry of Health, Labour and Welfare qualification for “sale of goods” or “service provision” at grades A-D with eligibility in the Kanto-Koshinetsu region.
(4) Must prove license for general sale of pharmaceuticals under the Pharmaceutical and Medical Device Act.
