1. Procurement Details
(1) Item Classification Number: 21
(2) Subject and Quantity: Complete set of LED lighting fixtures
(3) Specifications: According to the bidding instructions and specifications.
(4) Delivery Deadline: According to the bidding instructions and specifications.
(5) Delivery Location: According to the bidding instructions and specifications.
(6) Bidding Method: The bid amount must include all costs necessary to fulfill the requirements specified in (3). The winning bid price will be the bid amount plus 10% of that amount (fractions less than 1 yen will be truncated). Bidders must submit a bid form stating an amount equivalent to 100/110 of the estimated contract amount, regardless of consumption tax status.

2. Qualification for Participation
(1) Must have Ministry of Health, Labour and Welfare unified qualification for “sale of goods” at grade A, B, or C, and have qualification for the region where the hospital is located. Applications for qualification are accepted at designated offices as per government procurement notices dated March 31, 2026.
(2) Must not fall under Article 5 of the National Hospital Organization Contract Handling Regulations. Exceptions apply for minors or persons under guardianship with necessary consent.
(3) Must not fall under Article 6 of the Contract Handling Regulations.
(4) Must have qualifications designated by the responsible officer under Article 4 of the Contract Handling Regulations.

(4) Bid opening: October 8, 2026, 11:00 at the Medical Center Conference Room 1 & 2 (5F).
4. Other
(1) Language and currency for bidding and contracts: Japanese and Japanese yen.
(2) Bid and contract guarantees are exempted.
(3) Bidders must submit sealed bids with documents proving capability to fulfill the procurement by the deadline. Bidders must respond to explanations requested by the responsible officer before the bid opening.
(4) Bids from unqualified bidders or those failing obligations will be invalid.
(5) Contract document preparation is required.
(6) Award decision: The responsible officer will select the bidder who submits valid bids with required documents and offers the lowest price within the estimated price limits as per Article 21 of the Contract Handling Regulations.
